Back-to-School: Free immunizations, physicals for children in Broward

HOLLYWOOD, Fla. – Memorial Primary Care and Joe DiMaggio Children’s Hospital will host a Back-to-School Health Fair in Hollywood on Saturday.

School immunizations, vision and hearing screenings, and Broward County Schools Physical Examinations will be available at the Memorial Primary Care Center located at 4105 Pembroke Road in Hollywood on Aug. 3rd from 9 a.m. to 2 p.m.

The first 100 children are guaranteed to receive services.

Remaining children will be seen as time permits until 2 p.m. Immunizations and physicals are provided for Medicaid patients and children without health insurance, ages 4-7.

For the children to receive services, Parents/guardians must bring:

  • A copy of the child’s current immunization record is required for immunization
  • Parents/guardians must bring a valid photo ID or your child will not be eligible for services. If possible, also bring identification for your child such as a birth certificate, passport or school ID. If you won’t be with your children during the health fair, parents/legal guardians must complete the Kinship Letter to Treat form.
