Below is a list of places ordered shut last week by inspectors with the Florida Department of Business and Professional Regulation.

According to state records, no establishments were ordered shut in Miami-Dade and the Florida Keys last week.

“Roach activity present as evidenced by live roaches found. #1: Approximately 15 live roaches crawling behind reach-in cooler next to triple sink in kitchen. Operator sprayed area. #2: 5 roaches crawling on wall near pizza dough mixer in kitchen. Operator killed. #3: 1 roach crawling in from back door which has a gap near kitchen. Operator killed. #4: 1 live roach crawling on floor underneath flip top cooler in kitchen. #5: 2 live roaches crawling underneath double door reach-in cooler next to hand-wash sink in kitchen.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. #1: observed approximately 10 small flying insects on paper towel dispenser in kitchen. #1: observed 3 small flying insects landing on walls and rack holding clean dishes in kitchen #3: approximately 10 small flying insects landing and hovering on pot on top of pizza dough mixer in kitchen. #4: 1 small flying insect flying throughout kitchen.”

“Dead roaches on premises. Observed 2 dead roaches under soda machine at front counter at cook line area. Observed 1 dead roach in light shield above front counter.”

“Observed Accumulation of black/green mold-like substance in the interior of the ice machine/bin.”

“Food with mold-like growth. See stop sale. Observed 4 packets of moldy romaine lettuce.”

“Employee touched face/hair and then engaged in food preparation, handled clean equipment unwrapped forks or utensils, or touched unwrapped single-service items without washing hands for guests self-serve. Inspector had employees throw away forks and wash hands.”

“Open condiments provided for self-service not properly protected. Observed employee handling open unwrapped forks touching food contact surfaces placing in trays for guests to use.”

Observed interior of nozzle holders located server soda machine with mold like slime substance.”

Observed old moldy lettuce on barrier shelf of reach in cooler with possible cross contamination of old moldy lettuce dropping into fresh lettuce each time drawer is pulled open and closed.”

Canadian bacon (52°F - Cold Holding); butter (54°F - Cold Holding); sausage burritos (52°F - Cold Holding); liquid egg (53°F - Cold Holding) In reach-in cooler on cook-line, observed gasket to be broken thus causing cooler door to not close entirely. Ambient temperature of cooler is 54°F. Operator states all items were placed in cooler less than 3 hours ago. Operator moved items to walk-in cooler for proper cold-holding. Operator fixed gasket at time of inspection and temperature was going down at time of inspection.”
