MIAMI – State inspectors found roach infestations at an IHOP in Miami and Hungry Howie's in Lauderhill last week.

Both places were ordered shut.

Reports show there was mold in the ice machine and on the soda dispensing nozzles at the IHOP as well.

Siam Palace Shiva on Sunset Drive was ordered shut for the second time last week as well.

Roaches were spotted crawling on top of a food prep table.

Below is a list of places and some of their violations. All the places listed were allowed to re-open following ordered clean ups and re-inspections.
