Rodent droppings, rub marks, gnawed food, unsanitary conditions force inspectors to take action

Below is a list of places recently inspected by the Florida Department of Business and Professional Regulation. Some places were ordered to shut, and a follow-up inspection was ordered at others.

However, no eating establishments were ordered to shut down last week in Miami-Dade. This is the second week in a row this has occurred. How is it possible that there are no roach or rodent issues in a county with so many eating establishments?

All the places that were ordered to shut were allowed to reopen following a required cleanup and a re-inspection.

“Stop Sale issued due to food not being in a wholesome, sound condition. Sugar packets in dry storage room, open to kitchen, with gnaw marks through packet.”

“Roach activity present as evidenced by live roaches found. 1 live roach between chest freezer and stand freezer in dry storage. 1 live roach in orange storage container with dry pasta. 2 live on threshold of exterior door next to restrooms.”

“Dead roaches on premises. 2 dead under shelving in dry storage room open to kitchen. 3 dead under dunnage rack with dry pasta storage in storage room open to kitchen.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 4 live on dining room wall. 1 live on storage bin in dry storage open to kitchen. 3 live on exterior back door frame next to restrooms.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Prep room - 2 door Turbo Air - beans (55F); raw fish (51F); cooked ribs (51F). Per operator items placed in unit less than 3 hours ago. Observed damaged/missing gaskets and exposed insulation on doors. Operator moved items to walk in to quick chill. Cookline - left flip top - sliced turkey (55F); sliced roast beef (55F). Per operator items held in unit approximately 2 hours. No items prepped or portioned today. Observed damaged/missing gaskets on doors.”

“Employee began working with food and handling clean equipment without first washing hands. Cook came in from outside, put on gloves and began to fry chicken wings without first washing hands.”

“Floor soiled/has accumulation of debris. Under all shelving in walk in cooler.”

“Grease on the ground and/or pad around grease receptacle - in parking lot.”

“Dry storage room - Exterior door has a gap at the threshold that opens to the outside. Repeat Violation.”

“No plan review submitted and approved - renovations were made or are in progress.”

“Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again.”

“Proof of required state approved employee training not available for some employees.”

“Operating with an expired Division of Hotels and Restaurants license.”

“Dead roaches on premises. Approximately 10 dead roaches in cabinets with bag in box syrups in dining area. Approximately 2 dead roaches under dish machine in kitchen. Approximately 2 dead roaches in employee restroom next to triple sink. Approximately 2 dead roaches in storage cove in kitchen with bottle drink storage. Operator began removing dead roaches and cleaning the areas during inspection. Repeat Violation.”

“Objectionable odors in area where triple sink is located. Accumulation of water and debris in this area.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Magnetic knife rack with clean knives visibly soiled.”

“Floor area(s) covered with standing water. By water heater next to employee restroom. Under and around dish machine.”

“Floor soiled/has accumulation of debris. Under all shelving in dry storage open to kitchen. Under all cooking equipment in kitchen. Repeat Violation.”

“Cardboard used on floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first. Soiled cardboard in employee restroom. Soiled cardboard in back storage room open to kitchen. Soiled cardboard in front of walk in cooler.”

“Rodent activity present as evidenced by rodent droppings found. 1. Observed approximately 16 rodent droppings in dry storage area around water heater and behind washer and dryer. 2. Observed 6 rodent droppings under counter at sushi prep area. 3. Observed 2 rodent droppings under drain board in ware washing area. Operator cleaned areas at the time of inspection.”

“Stop Sale issued due to food not being in a wholesome, sound condition. Container of raw pre-shucked clams/mussels/oysters does not have a proper label. See stop sale. Observed raw, shucked oysters on metal pans in walk in freezer without any labeling or tags attached.”

“Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed tags stored in plastic jug not in chronological order according to the last date they were served in the establishment.”

“Raw animal food stored over or with unwashed produce. Observed container with packaged raw beef stored on shelf above bagged raw onions in walk in cooler. Repeat Violation.”

“Floor tiles missing and/or in disrepair. Observed cracks and holes in floor tile throughout kitchen, prep, and ware washing areas. Repeat Violation.”

“Roach excrement and/or droppings present. Observed large buildup of roach droppings inside of not in use 1 door white reach in freezer.”

“Container of medicine improperly stored. Prescription medication stored on shelf above cook line with single service items, operator removed.”

“No soap provided at hand wash sink. Observed at front counter, kitchen, prep area, ware wash in area, hand wash sinks. Repeat Violation.”

“No proof of required state approved employee training provided for any employees.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sour cream (67F - Cold Holding) at cook line. As per operator for 30 mins. Operator placed sour cream inside reach in cooler.”

“Bowl or other container with no handle used to dispense food. Observed plastic cup with no handle stored inside container of rice in dry storage. Repeat Violation.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents above server soda station heavily soiled with dust. Repeat Violation.”

“Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed ice bucket stored not inverted on top of ice machine. Repeat Violation.”

“Equipment in poor repair. Observed gaskets broken on walk in freezer door and reach in freezer near cook line. Repeat Violation.”

“Food stored on floor. Observed boxes of snow crab stored on floor inside walk in freezer. Observed boxes of eggs stored on floor inside walk in cooler.”

“Hand wash sink used for purposes other than handwashing. Observed sponge stored inside hand sink next to cook line. Repeat Violation.”

“Required employee training expired for some employees.”
