Rodent issues found in South Florida pizzeria and diner

LBTS diner stays closed to properly address issues found in inspection

Below is a list of places that were ordered shut by the Florida Department of Business and Professional Regulation last week.

No places in Miami-Dade and the Florida Keys were ordered shut last week.

All the places mentioned were allowed to re-open following an ordered cleanup and then a re-inspection.

Storage unit is separate from kitchen and contains a reach in cooler, reach in freezer, and dry storage items- pizza boxes 2.Approximately 4 rodent droppings in hallway leading to triple sink under dunnage wracks containing preformed pizza boxes.ā€

ā€œLive, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Presence of insects, rodents, or other pests. 1. Approximately 5 small flying insects in hallway leading to dry storage containing triple sink 2. Approximately 5 small flying insects in kitchen- between hand wash sink and flip top cooler at cook line.ā€

ā€œAccumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Accumulation of dead flies on fly sticky trap in kitchen below cook line hand wash sink.ā€

ā€œObjectionable odors in bathroom or other areas of the establishment. Observed objectionable odor in hallway with bathroom leading storage room with triple sink.ā€

ā€œCeiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Accumulation of dust on kitchen AC vents.ā€

ā€œNonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood vent in kitchen soiled with dust at cook line.ā€

ā€œApproximately 10 rodent droppings in exterior storage unit. Operator removed remaining rat droppings. Operator cleaned and sanitized the area. 2. 0 rodent droppings in hallway leading to triple sink under dunnage wrack containing preformed pizza boxes.ā€

ā€œRodent activity present as evidenced by rodent droppings found. Approximately 24 rodent droppings in kitchen on shelf containing audio equipment. Approximately 12 rodent droppings under microwave on shelf in kitchen. Approximately 4 rodent droppings on prep table under and behind steam table in kitchen. Approximately 2 rodent droppings on top of dish washing machine in kitchen. Approximately 2 rodent droppings located in rear storage area by water heater.ā€

ā€œDead roaches on premises. 1 dead roach under triple sink. 1 dead roach next to stove in kitchen. 1 dead roach on shelf at front counter.ā€

ā€œAccumulation of debris on exterior of ware washing machine.ā€

ā€œFloor soiled/has accumulation of debris. Behind stove in prep room and standing water on floor behind ice machine.ā€

ā€œStop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. 2x 6oz ROP Mahi mahi in Cookline flip top still in ROP package, per operator item was taken out of freezer yesterday and held in unit overnight.ā€

ā€œTime/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Expo area-butter (70F). Observed item held in an ice bath with ice water solution too low. Per operator item out for 1 hr., operator placed item in reach in to quick chill.ā€

ā€œNo proof of required state approved employee training provided for any employees.ā€

ā€œTime/temperature control for safety food thawed in an improper manner. Observed at triple sink packages of cheese thawing at room temperature.ā€

ā€œStop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed at pizza station cooler - ham (47F - Cold Holding); grilled peppers (47F - Cold Holding); sausage (47F - Cold Holding); Brussels sprouts (47F - Cold Holding); tomato sauce (47F - Cold Holding). As per operator items not portioned or prepared today. As per operator, items held out of temperature over 4 hours.ā€

ā€œDish machine chlorine sanitizer not at proper minimum strength. Discontinue use of dish machine for sanitizing and set up manual sanitization until dish machine is repaired and sanitizing properly. Observed dishwasher at 00ppm after 3 attempts.ā€

ā€œEmployee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed at dishwasher - employee handled soiled dishes then proceeded to cut cheese without washing hands.ā€

ā€œServer handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed at dishwasher - employee handled soiled dishes then proceeded to cut cheese without washing hands. Educated employee on handwashing techniques.ā€

ā€œCeiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. -Observed accumulation of black mold like substance buildup on AC vents and ceiling tiles throughout kitchen.ā€

ā€œEquipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. -Observed operator wash and rinse food containers and utensils without sanitized. Advise operator to set up 3 compartment sink properly.ā€

ā€œNonfood-contact surface soiled with grease, food debris, dirt, slime or dust. -Observed accumulation of brown like substance buildup on can opener holster. Repeat Violation.ā€

ā€œFood-contact surface soiled with food debris, mold-like substance or slime. -Observed accumulation of black mold like substance buildup on can opener blade. Repeat Violation.ā€

ā€œNo currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Manager returned during inspection. Repeat Violation.ā€

ā€œStop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. 1)Kitchen flip top; cooked sausage (54F - Cold Holding); shredded lettuce (50F - Cold Holding) per operator foods in unit overnight. Foods not portioned or prepared today. 2)On kitchen counter; cooked chicken (80F at 1200pm - Heated Cooling since 9am) did not cool to 70F in 2 hours.ā€

ā€œFood being cooled by non- approved method as evidenced by inadequate rate of cooling during time of inspection. cooked beef (69F at 1200pm to 60F at 1225pm - Heated Cooling since 09am) observed food item left on counter cooling in room temperature.ā€

ā€œUnnecessary persons in the food preparation, food storage, or ware washing area. Observed a small child walking through the kitchen area when food is being prepared. Instructed operator to keep child away from prep area. Now sitting in to go cashier station.ā€
