Rodent issues found at 4 South Florida restaurants

Below is a list of places that were inspected by the Florida Department of Business and Professional Regulation last week.

Some were ordered shut while others were issued a “follow-up inspection required.”

No places in Miami-Dade and the Florida Keys were ordered shut last week.

All the places ordered shut were allowed to re-open following an ordered cleanup and re-inspection.

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. -one flying insect flying around ice cream cooler by front counter- between coffee and deli station at flying insects flying around prep station, in rear kitchen -rear kitchen. -one flying insect flying around dry good storage area, in rear kitchen.”

“Food stored outside. Enclosed cooler with washed vegetables cans and gallons of food, stored next to dumpster area.”

“Inside/outside of dumpster not clean. Soil , flies and food debris inside dumpster.”

“Floor soiled/has accumulation of debris. -outside cooler- substantial buildup of food debris -front kitchen, rear kitchen, dry goods storage area, under cooking equipment.”

“Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Rear cook line area .Employee came from outside, wear gloves to continue preparing food without washing hands. After manager instructed, employee discarded gloves, washed hands and put in new pair of gloves.”

“Dish machine chlorine sanitizer not at proper minimum strength. Discontinue use of dish machine for sanitizing and set up manual sanitization until dish machine is repaired and sanitizing properly. Dishwasher (Chlorine 00ppm).”

“Employee washed hands in a sink other than an approved hand wash sink. Employee washed hands on prep sink in cook line, after manager instructed employee used hand wash sink.”

“Hand wash sink not accessible for employee use at all times. Hand wash sink on second cook line not available for employees to wash hands. Not water supply.”

“Rodent activity present as evidenced by rodent droppings found. Seventeen rodent dropping under small hand sink by floor mixer in far corner of kitchen farthest from cook line and dry storage. Employee removed droppings, swept, and mopped area.”

“Time/temperature control for safety food thawed in an improper manner. Container of fish thawing on back prep table thawing at room temperature. Employee moved fish to walk in cooler.”

“Floor area(s) covered with standing water. Standing water on floor under hand sink and triple sink at far end of kitchen.”

“Floor soiled/has accumulation of debris. Floor area under cook line has debris and grease buildup. Repeat Violation.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Floor mixer bowl has food debris buildup.”

“Cove molding at floor/wall juncture broken/missing. Cove molding tiles damaged under small hand sink at front corner of kitchen and along wall near cookline. Repeat Violation.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Grease buildup on hood filters. Repeat Violation.”

“Plumbing system in disrepair. Drain lines under hand sink and triple sink at far end of kitchen leaking. Repeat Violation.”

“Standing water or very slow draining water in hand wash sink, three-compartment sink, or mop sink. Hand sink at far end of cook line slow draining / standing water.”

“No handwashing sign provided at a hand sink used by food employees. No handwashing sign at front counter hand sink.”

“Employee with no hair restraint while engaging in food preparation. Multiple employees in kitchen not wearing hair restraints.”

“Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee left cook line to mop floor area and move items to cooler and returned to, cook line without washing hands before putting on gloves. Reviewed proper procedures with employee and manager and employee washed hands.”

“Rodent activity present as evidenced by rodent droppings found. -Observed 3 rodent droppings behind linen cabinet. Linen cabinet is located next to mop sink . -Observed 2 rodent droppings underneath table in dishwashing area. -Observed 2 rodent droppings at service station next to cook line.”

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin. -Observed accumulation of brown like substance buildup inside ice machine. Repeat Violation.”

“Wall soiled with accumulated grease, food debris, and/or dust. -Observed accumulation of old food residue buildup on walls at prep stations.”

“Buildup of food debris/soil residue on equipment door handles. -Observed accumulation of old food residue buildup on double doors refrigerator at cook line.”

“Cutting board has cut marks and is no longer cleanable at cook line. Repeat Violation.”

“Roach activity present as evidenced by live roaches found. 3 live roaches in Premium Levella reach in cooler entrance of storage area next to kitchen. Cooler not in use.”

“Stop Sale issued on time/temperature control for safety food due to temperature abuse. Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Vegetables reach in- ; Cooked Chicken (62F - Cooling). Per Cook food item in unit overnight. Item not prepared or portioned today. Observed water condensation on lid of container.”

“Exterior door has a gap at the threshold that opens to the outside. Repeat Violation.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw fish over cooked chicken wings in ikon reach in. Repeat Violation.”

“Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw shell eggs over raw fish in ikon reach in cooler.”

“Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cut tomatoes (55F - Cold Holding); cut iceberg (55F - Cold Holding) in shallow pans in flip top cooler per operator cooling since 10:00am did not cool within 4 hours.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Shelves over flip top cooler in kitchen surface soiled with food debris, mold-like substance or slime.”

“Bowl or other container with no handle used to dispense food. Observed bowl or other container with no handle used to dispense rice ,dried beans and corn meal. Repeat Violation.”

“Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee talking on cell phone and then engaged in food preparation without washing hands.”

“Hand wash sink used for purposes other than handwashing. Observed wet wiping cloths in hand washing sink in kitchen.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Food not prepared or portioned today. 3 cooling drawers in cook line-; mix shredded cheese (67F - Cold Holding); sliced yellow cheese (52F - Cold Holding); pepper jack (47F - Cold Holding); sliced American cheese (46F - Cold Holding), per employee food on unit less than 4 hours, advised operator to add ice for a quick chill and to move all time temperature control food items to a working cold holding unit, until technician verified proper cold holding temperatures.
