Live rodent, worms, roaches found inside South Florida establishment

Below is a list of places that were ordered shut last week by inspectors with the Florida Department of Business and Professional Regulation.

All the places mentioned were allowed to re-open following an ordered cleanup and re-inspection.

16 VIOLATIONS FOUND

“Rodent activity present as evidenced by rodent droppings found. Observed approximately 10 rodent droppings on top of the dishwasher machine, at the kitchen area.”

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Repeat Violation.”

“Accumulation of debris on exterior of ware washing machine.”

“Food stored on floor. Observed raw steak container stored on walk in cooler floor. Repeat Violation.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Alfredo sauce (132F - Hot Holding); marinara sauce (132F - Hot Holding); spinach dip (118F - Hot Holding); key lime butter sauce (101F - Hot Holding) at cook line, as per employee less than 3 hours. Employee started to reheating food.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed soiled can opener blade. Employee wash can opener during inspection.”

“Hand wash sink used for purposes other than hand washing. Observed dirty container inside HWS at cook line.”

“Roach activity present as evidenced by live roaches found. 1 live roach in hand sink next to 3 compartment sink in kitchen 3 live roaches in unused reach in cooler in kitchen by back door. 5 live roaches behind hostess stand in electrical box and loose piece of wood, behind file cabinet, on other side of kitchen wall.”

“Roach excrement and/or droppings present. Approximately 20 roach egg sacks in unused Reach in cooler by back door in kitchen.”

“Dead roaches on premises. 1 dead roach in service station 1 dead roach under reach in cooler in kitchen. 1 dead roach in back storage.”

“Time/temperature control for safety food thawed in an improper manner. 2 large turkeys defrosting at room temperature on counter in kitchen.”

“Floors and walls soiled/has accumulation of debris. Under all equipment in kitchen and service areas.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Sides of equipment, and cabinets throughout establishment soiled. Repeat Violation.”

“Screen in door torn/in poor repair - vermin present. Screen door is off hinges and only leans against door opening in kitchen.”

“Hand wash sink not accessible for employee use at all times. Food pan in hand wash sink.”

“Perimeter walls and roofs do not effectively protect establishment against environmental cross contamination or the entrance of pests. Large hole in bottom of wall outside of back door to kitchen, that exposes the interior wall to the kitchen. Buildout of wall, creating storage room, behind back storage with area exposed to exterior by roof line. Operator has canned goods stored in this room.”

“Live rodent present. Observed live mouse running behind stove and get into hole in the wall.”

“Rodent activity present as evidenced by rodent droppings found. Observed more than 25 mouse droppings behind stove next to hole on the wall. And 3 more droppings under 3 compartment sink.”

“Roach activity present as evidenced by live roaches found. Observed one live roach in the kitchen area. Operator killed it and discarded.”

“Dead roaches on premises. Observed one dead roach exit the door. And one dead roach under preparation table at kitchen area.”

“Dead worms present. Observed 2 dead worms by the exit back door.”

“Pesticide/insecticide labeled for household use only present in establishment. Observed 2 fly mosquitoes killer cans on top of employee hand washing sink.”

“Hole in or other damage to wall. Observed a hole on the wall behind the stove by where the mouse enters and exit from preparation area. And hole on the wall next to exit door at the back of the kitchen. Operator started to put foam sealants on the holes.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed floor fan and fan over reach in cooler soiled.”

“Roach activity present as evidenced by live roaches found. Approximately 4 live roaches found behind the fryer on the cook line.”

“Buildup of food debris/soil residue on equipment door handles. On the True reach in cooler door handles.”

“Raw animal food stored over/not properly separated from ready-to-eat food. In the walk in cooler, raw eggs stored over raw fish.”

10 VIOLATIONS FOUND

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 6 flying insects flying around and landing on wall and plastic containers in dish washing area.”

“Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed cook using bare hands to plate quesadillas.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw lobster stored on top of chocolate chip cookie in reach in freezer.”

“Dish machine chlorine sanitizer not at proper minimum strength. Discontinue use of dish machine for sanitizing and set up manual sanitization until dish machine is repaired and sanitizing properly. Observed 0ppm. No three compartment sink in establishment. Establishment does not have any other means of sanitizing equipment.”

“Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked wing prepared more than 24 hours prior to inspection not date marked. Per operator item was prepared approximately 3:00 pm the day prior to inspection.”
