Dead roach floating in sink with thawing fish forces restaurant closure

Below is a list of places ordered shut by inspectors with the Florida Department of Business and Professional Regulation last week.

All the places mentioned below were allowed to re-open following an ordered clean-up and re-inspection.

According to state records, no restaurant kitchens were ordered shut in Miami-Dade County last week.

“Stop Sale issued due to food not being in a wholesome, sound condition. Observed 1 dead roach inside the three compartment sink with 2 bags of hog fish thawing, submerged under standing water.”

“Three-compartment sink used for ware washing not cleaned before use as a food preparation sink. Observed 1 dead roach inside the three compartment sink with 2 bags of fish thawing, submerged underneath standing water.”

“Dead roaches on premises. Observed 1 dead roach inside the three compartment sink with bags of fish thawing.”

“Time/temperature control for safety food thawed in an improper manner. Observed in the ware wash area: 2 bags of fish thawing in standing water. As per operator, placed in the three compartment sink 45 minutes before the inspection. Observed 1 dead roach inside the three compartment sink with bags of fish thawing. Operator discussed with the operator the proper methods for thawing. Observed operator discard the 2 bags of fish during.”

“Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on the wall by the circuit breaker.”

“Food with mold-like growth. See stop sale. Walk in cooler by the dish machine: mold-like growth on 6 strawberries.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed in the dry storage area: 2 live flies resting in a storage container with grits. Observed 9+ live flies flying around a prep table and resting on containers of pickled spice and black pepper in the ware wash area. Observed on the prep table in the ware wash area: 2 live flies on bags of bread. Observed approximately 6 live flies on clean pans and shelf above the three compartment sink. Kitchen, shelf above flip top reach in cooler: approximately 3 live flies resting on clean pan, 2 live flies on a roll of unwrapped paper towels, 2 live flies on a box of wraps, approximately 6 live flies on top of to-go containers, 3 live flies on a cord.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed in the kitchen on a prep table in the ware wash area: can opener blade soiled.”

“In-use tongs stored on soiled surface between uses. Observed in the kitchen by the cook line: a pair of tongs hanging on the garbage can with trash inside. Observed operator place tongs in the three compartment sink to be cleaned.”

“Hand wash sink used for purposes other than hand washing. Bar area: observed employee throw ice from a cup into the hand washing sink”

“Roach activity present as evidenced by live roaches found. 1 live roach observed in the back of kitchen on the door frame of office door located inside kitchen. 1 live roach on floor by walk-in cooler door. Operator killed roach and sanitized area.”

“Dead roaches on premises. 3 dead roaches observed in the back of the kitchen behind the speed rack located by office door. 4 dead roaches observed by the hot box unit at the back of the kitchen near the ware washing area.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 2 flies observed flying around and landing on unwashed limes, flip-top lid, and inverted to go containers along the cook line. Operator killed both and sanitized area.”

“Raw animal food not separated from ready-to-eat food during preparation. Observed in the 2 door reach in freezer located by office door raw chicken over fries. Observed frozen seafood over frozen vegetables.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Observed in the 2 door lowboy across from the fryer a plate of raw chicken above open containers of ready to eat items. Operator rearranged to move chicken below.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed slicer on prep table in kitchen soiled with food debris.”

“Old food stuck to clean dishware/utensils. Observed food debris on clean plates on expo line. Operator sent to wash and sanitize.”

“Hand wash sink used for purposes other than hand washing. Observed hand wash sink by wait station with soiled cutting knives in it. Observed hand wash sink in kitchen with plastic bucket in it. Operator removed to wash and sanitize.”

“Manager or person in charge lacking proof of food manager certification.”

“No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.”

“Required employee training expired for all employees.”

“Dead flies on premises. -Observed 38 dead flies at bar tap counter.”
